The Adult ADHD Self-Report Scale (ASRS-v1.1) was developed by the WHO with Kessler et al. (2005). It's the most widely used adult ADHD screen. Below are sample items covering both inattention and hyperactivity-impulsivity domains.

Answer: Often or Very Often → screens positive on this item

2. How often do you have difficulty getting things in order when you have to do a task that requires organization?

3. How often do you have problems remembering appointments or obligations?

4. When you have a task that requires a lot of thought, how often do you avoid or delay getting started?

Inattention. Probes task initiation and procrastination.
5. How often do you fidget or squirm with your hands or feet when you have to sit down for a long time?

Hyperactivity. Probes physical restlessness.
6. How often do you feel overly active and compelled to do things, like you were driven by a motor?
Answer: Often or Very Often → positive

How It's Scored

The ASRS-v1.1 short-screen uses 6 items. If you score 'often' or 'very often' on 4 or more items, the screen is positive — meaning your responses are consistent with adult ADHD and clinical assessment is the recommended next step. The full 18-item ASRS provides per-symptom severity ratings.
